1. How to request a record
- Use Form 2.Download the current Request for Access form from the Information Regulator's PAIA forms page.
- Explain the right.For a private body, identify the right you seek to exercise or protect and explain why the requested record is required.
- Send it to the Information Officer.Email the completed request to legal@companybrain.work, or deliver it to the physical address above.
- Receive a written decision.We decide requests within 30 days, subject to a permitted extension of up to a further 30 days.
A person unable to read or write, or with a disability, may make an oral request; the Information Officer will reduce it to writing and provide a copy.

Informal access remains subject to confidentiality obligations owed to third parties. A formal PAIA request may be needed where it cannot be supplied informally.
3. Categories of records held
Order forms, proposals, scope sheets, quotations, statements of work, NDAs and commercial correspondence.
Source code, architecture, specifications, test reports, defect registers, releases and deployment records.
Environment and connector configuration, support correspondence, incident records, service reviews and usage statements.
This manual, privacy notices, consent and request records, security-compromise records and remediation material.
Website material, analytics reports, invoices, tax records, supplier agreements and business correspondence.
4. Fees

Where access is granted, further access fees may apply for reproduction and preparation. A deposit may be required where search and preparation are estimated to exceed six hours. The official, complete fee schedule is in the issued manual.
5. Personal information
Company Brain processes personal information to manage business relationships, conclude and perform agreements, provide support, run consent-based website analytics, meet accounting and legal obligations, and protect systems from fraud and abuse. The customer is the responsible party for content processed in a customer-controlled Company Brain environment; Company Brain acts as an operator only as agreed for deployment, configuration, maintenance and support.
The complete manual describes categories of data subjects, recipients, cross-border transfers and security measures. It also states the limits: Company Brain holds no security certification and has not had an independent penetration test.
6. Remedies and regulator
If you are dissatisfied with a decision, you may lodge a PAIA complaint with the Information Regulator using Form 5 within 180 days, or apply to a court for appropriate relief within 180 days. Internal appeal procedures apply to public bodies, not private bodies.
7. Availability and review
This manual is available on this page, for public inspection at the principal place of business during normal business hours, and from the Information Officer on request. It will be reviewed at least annually and whenever there is a material change to Company Brain's records, processing, providers, contact details or applicable law.
